EFFICACY OF COUPLE THERAPY AS A TREATMENT FOR DEPRESSION: AN UPDATED META-ANALYSIS

The authors present the results of a meta-analysis of a couple therapy as a treatment for depression realized within the framework of Cochrane Collaboration. Thirteen controlled trials, published between 1989 and 2012, involving 508 subjects, were included. Various treatment models (cognitive-behavioral, emotion-focused, systemic) were compared with individual psychotherapies of well-known efficacy, drug therapy or no treatment/waiting list. No difference was found on depressive symptoms between couple therapy and individual psychotherapy. Relationship distress, however, was significantly reduced by couple therapy. Too few data are available for comparisons with drug therapy and no treatment. No indications of advantages for a specific model can be drawn from the trials. The findings in most studies are weakened by heterogeneity, small sample sizes, assessments at post-treatment only, unclear sample representativeness. The role of change in couple satisfaction as a mediating variable in depression improvement relationships has not been adequately tested. Although no advantages for couple therapy as a treatment for depression in comparison to individual therapy have been found, evidence for improvement in relationships may favor the choice of couple therapy when relational distress is a major problem or attention to relational quality is warranted.
