OPEN DIALOGUES: NEW PARADIGM IN MENTAL HEALTH WORK FOR PSYCHOTIC AND OTHER SEVERE PROBLEMS

Open dialogue is a psychiatric practice first initiated in the Finnish Western Lapland province. After starting the work late 80’s in several studies on psychosis, depression and non-defined problems the outcomes have been extraordinary. E.g. in first episode psychosis more than 80% did not have any remaining psychotic symptoms and more than 80% had returned to full employment in both 2 and 5 years follow-up. This was received in the practice where antipsychotic medication was used in about 1/3 of the patients meaning that most part did not have antipsychotic medication. In the latest studies it was noted that in Western Lapland province the incidence of schizophrenia has declined to one tenth from the middle of 80’s.
The main guidelines for the Open Dialogue practice are organizing crises intervention service and guaranteeing immediate help, always first working with the family and other social network, integrating different therapy methods based on the unique needs of the families and patients, guaranteeing availability and psychological continuity and by increasing safety for tolerating the uncertainty in the crisis. Making all this possible the joint open meeting focus mainly on generating dialogue between the participants to make all the voices heard. In the presentation first the main principles of Open Dialogue will be described and reflected on what are the basic elements that contribute to such good outcomes. Out of these elements at least immediate help, polyphony of the social network, avoiding or minimizing the antipsychotic medication and generating dialogue to hear all the voices should be present at the same time to guarantee optimal outcome.
